Ages 11–20 — $60/month
Ages 21–64 — $80/month
$100 One-Time Enrollment Fee
✓ Unlimited visits included with membership
NON-MEMBER SERVICES
In-Person Sick Visit — $100
Includes in-office testing when indicated
Virtual Sick Visit — $75
Annual Physical — $100
Annual Physical + Pap — $150
Sports Physical — $45
Additional Services
LABS, IMAGING & PRESCRIPTIONS
These services are not included in the office visit price; however, your insurance may be used.
Labs via Labcorp
Imaging can be sent to Wake Radiology or Raleigh Radiology
Prescriptions are sent to Pharmacy of your choice
