PRIMARY CARE MEMBERSHIP
Ages 11–20 — $60/month
Ages 21–64 — $80/month
$100 One-Time Enrollment Fee
✓ Unlimited visits included with membership, providing comprehensive primary care services, including annual physicals and more.
NON-MEMBER SERVICES
In-Person Sick Visit — $100
Includes in-office testing when indicated, perfect for urgent care needs.
Virtual Sick Visit — $75
Take advantage of our telehealth services for convenient virtual care.
Annual Physical — $100
Ensure your health with a thorough annual physical examination.
Annual Physical + Pap — $150
Get both your annual physical and Pap test in one visit.
Sports Physical — $45
Prepare for athletics with our focused sports physical.
Additional Services
LABS, IMAGING & PRESCRIPTIONS in Primary Care: These services are additional to the office visit price, but you can use your insurance. For physical assessments, labs are performed via Labcorp. Imaging results can be forwarded to Wake Radiology or Raleigh Radiology, depending on your preference. For your prescriptions, they can be sent to the pharmacy of your choice, whether you are utilizing our telehealth services or in-person urgent care.
